Job description: Hi All I am looking for Client Liaison Officer to work with Home Care Provide based in Cambridge.



An award nominated Home Care provider that offers Live in care service enabling our clients to remain at home for longer. We currently have an opening for an Adult Care Assessor to join our team.
Duties
*Arranging hospital discharge including arranging for health care equipment
*Producing Customer support plans
*Complete care needs assessment including noting what the client's requirements are
*Liaising with GP's and Health Care professionals on behalf of the client
*Providing Case Management support for your specific client group
*Being a point of contact for general health questions for the Community Carers and practitioners.
*Carrying out observations and spot checks of carers practice
*mange customer complaints
*Make regular quality care calls to customers and stakeholders
*Act as a Catalyst for change and improvement in performance and quality

To apply for this role:
*You will have excellent communication skills both verbal and written
*You will demonstrate excellent time management skills
*You will be confident in working independently and as part of a team
*You will need to have the ability to engage effectively with a variety of professionals, carers and clients.
*Driving license
