Job description: Adult's Social Work Assistant - Hospital Team - Shrewsbury

Service Care Solutions are recruiting for 2 Social Work Assistants to work in the Hospital Team with Shropshire County Council.

This role would be working at the hospital supporting vulnerable adults ready for discharge and is an initial 3 month contract with great opportunities for extension and progression!

Key features of this role involve:

*Service delivery that is person centred and outcome focussed with an emphasis on encouraging independence and enabling recovery
*Collaborative work with colleagues and with other professionals and agencies with an emphasis on a multi-agency team of support being in place to work with individuals whose needs are complicated or unstable
*Multi agency, Social Worker led, investigation of safeguarding concerns involving vulnerable adults and the development and maintenance of adult support plans
*Implementing care plans and commissioning support

Essential requirements for this role include:

* Previous placements/work at Sheopshire Council is HIGHLY desirable
* Car driver preferable
* Previous experience working with Adults
