Locum Social Worker – Hospital Discharge to Assess (South Team)
Manchester City Council

Are you an experienced and dedicated Social Worker looking for a rewarding locum opportunity in a dynamic and supportive team? Manchester City Council is seeking a committed professional to join the Hospital Discharge to Assess South Team working through Service Care Solutions.

As part of this fast-paced and vital service you will:
  • Support hospital discharges assessing and reviewing the care needs of citizens transitioning out of hospital.
  • Complete Care Act assessments and Mental Capacity assessments as required.
  • Work across all hospital discharge pathways including:
    • Reviewing care packages in the community
    • Discharges from intermediate care beds
    • Citizens discharged to Pathway 3 (e.g. residential and nursing care settings)
  • Contribute to crisis response work helping prevent unnecessary hospital admissions.
  • Collaborate with the Hospital at Home service to ensure safe timely discharges and community support.
  • Participation in a 1-in-4 weekend rota required (supportive team environment).
  • Flexible hybrid working considered following induction and service need.
✅ Requirements:
  • Qualified Social Worker (SWE registered)
  • Experience in hospital discharge / adult social care settings
  • Strong working knowledge of the Care Act, MCA and hospital pathways
  • Able to work autonomously and manage a busy caseload
