Are you an experienced and dedicated Social Worker looking for a rewarding locum opportunity in a dynamic and supportive team? Manchester City Council is seeking a committed professional to join the Hospital Discharge to Assess South Team working through Service Care Solutions.
As part of this fast-paced and vital service you will:
- Support hospital discharges assessing and reviewing the care needs of citizens transitioning out of hospital.
- Complete Care Act assessments and Mental Capacity assessments as required.
- Work across all hospital discharge pathways including:
- Reviewing care packages in the community
- Discharges from intermediate care beds
- Citizens discharged to Pathway 3 (e.g. residential and nursing care settings)
- Contribute to crisis response work helping prevent unnecessary hospital admissions.
- Collaborate with the Hospital at Home service to ensure safe timely discharges and community support.
- Participation in a 1-in-4 weekend rota required (supportive team environment).
- Flexible hybrid working considered following induction and service need.
- Qualified Social Worker (SWE registered)
- Experience in hospital discharge / adult social care settings
- Strong working knowledge of the Care Act, MCA and hospital pathways
- Able to work autonomously and manage a busy caseload
