Locum Adults Social Worker – Preparing for Adulthood (PFA)
Rotherham Metropolitan Borough Council
£33.12 per hour (via Oscar Morgan-Done at Service Care Solutions)
6 Months Fixed Term

I am currently recruiting a dedicated and experienced Locum Adults Social Worker to join Rotherham MBC’s Preparing for Adulthood (PFA) Team. This is a fantastic opportunity for a skilled practitioner who is passionate about supporting young people as they transition into adulthood. You’ll be working within a supportive team environment to ensure smooth and person-centred transitions for young adults with complex needs.

Key Responsibilities
  • Undertake Care Act Assessments to establish eligibility and develop tailored support plans.
  • Complete Focused Reviews and manage Direct Payments effectively and accurately.
  • Apply the Mental Capacity Act (MCA) to assess capacity and make Best Interests decisions.
  • Carry out and contribute to Continuing Healthcare Checklists collaborating with health partners as needed.
  • Provide robust support to young people transitioning from children's to adult services including care leavers and those with SEND.
  • Maintain high-quality, strengths-based casework ensuring legal and statutory duties are met.
Requirements
  • Qualified Social Worker with Social Work England registration.
  • Demonstrable experience in adult social care with a focus on transitions/young adults.
  • Strong knowledge of relevant legislation including the Care Act, MCA and CHC processes.
  • Ability to work autonomously and manage a complex caseload.
  • Commitment to working 3 days per week in-borough for office work and home visits.
