I am currently recruiting a dedicated and experienced Locum Adults Social Worker to join Rotherham MBC’s Preparing for Adulthood (PFA) Team. This is a fantastic opportunity for a skilled practitioner who is passionate about supporting young people as they transition into adulthood. You’ll be working within a supportive team environment to ensure smooth and person-centred transitions for young adults with complex needs.
Key Responsibilities
- Undertake Care Act Assessments to establish eligibility and develop tailored support plans.
- Complete Focused Reviews and manage Direct Payments effectively and accurately.
- Apply the Mental Capacity Act (MCA) to assess capacity and make Best Interests decisions.
- Carry out and contribute to Continuing Healthcare Checklists collaborating with health partners as needed.
- Provide robust support to young people transitioning from children's to adult services including care leavers and those with SEND.
- Maintain high-quality, strengths-based casework ensuring legal and statutory duties are met.
- Qualified Social Worker with Social Work England registration.
- Demonstrable experience in adult social care with a focus on transitions/young adults.
- Strong knowledge of relevant legislation including the Care Act, MCA and CHC processes.
- Ability to work autonomously and manage a complex caseload.
- Commitment to working 3 days per week in-borough for office work and home visits.
