Job description: Cardiff Council are looking for an experienced Adults Social Worker to join their Learning Disabilities team. This is a temporary, full time position with an immediate start available! In this role you would be working with Service Users and their families to maximise independence and opportunities which enable service users to enjoy purposeful, meaningful and fulfilled lives. Your role will include carrying out assessments, care planning, person centred planning and care management. You will work with individuals, families, carer groups and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning. You will also need to work positively with colleagues in the statutory and independent sector to ensure an appropriate multi-agency approach, and to empower clients to speak for themselves.

The ideal candidate will have:
*Social work degree, or equivalent
*HCPC or Care Council Wales registered
*Experience of working with adults with learning disabilities
*Full UK driving license and vehicle
