Job description: A rare and unique opportunity has arisen for an Assessment Officer with British Sign Language skills within the Doncaster area

We are currently recruiting for an Assessment Officer to work with Hearing Impaired to provide an initial assessment and interviewing service for older people, vulnerable adults and carers. The Assessment Officer will undertake where appropriate a Care Act (2014) Assessment as well as providing a care management service for individuals for people with hearing impairments in accordance with Council policies. This will in include carrying a caseload and using a robust performance management framework to ensure effective service delivery implementing innovative solutions by working across organisational boundaries to meet the long-term needs of the most complex Adults in Doncaster.

Core responsibilities and duties of this position includes:

* Promoting independence and reducing risks for adults living in Doncaster in line with the Care Act (2014) for people with hearing impairments
* Undertaking a person centred strengths based care act assessment to identify need taking the appropriate action whilst respecting individual values, cultures and diversity. During a strengths based assessment of need assessing and offering advice around technical equipment that will support independence in a person centred way
* Identifying needs and creating support plans to meet identified needs as well as determining eligibility for services in accordance with Care Act Guidance
* Management of a caseload acknowledging that some individuals may require assistance from other professionals
* Identification of risks or incidents of possible abuse including reporting this to the appropriate staff member in line with the Councils Safeguarding adult's policy and procedure
* Supporting and working with Qualified Social Workers and other professionals in cases which have become complex
* Assisting in the development and implementation of the locality based community led support model using creative ways to support service users and carers

Specialist skills and knowledge

* British Sign Language Level 3
* Awareness of working within a Community Led Support model
* Knowledge of adult health and social care provision, relevant statutory frameworks and policy including the health and social Care Act (2014) and other relevant legislation
* Experience of lone working including carrying out assessments in residents homes and community venues
* NVQ Level 3 in Social Care or equivalent
