Job description: A Band 4 Associate Practitioner/ Senior Support Worker is needed for a Mental Health Assertive Outreach Team within Grays for a short-term contract with guaranteed consistent work and competitive rates of pay!

Band 4 temporary pay rate: £14ph ltd

Hours and length of contract:
*Full Time 37.5 hours per week
*Monday to Friday 09:00 - 17:00
*Initial 3 month contract (with likelihood of extension)
*The manager requires the position to be filled ASAP!

Daily Duties:
Working as part of the Assertive Outreach Team within the Recovery and Wellbeing Team you will work regularly without direct supervision and be responsible for ensuring that the health and social care needs of patients are met.

Daily Duties include but are not limited to:
*assist in planning recovery based programmes of care and, together with qualified staff, implement these plans under the supervision of qualified staff
*Observe and report changes in the patient's health and environment to qualified staff and appropriate agencies. This would include patient's ability to self-care and deterioration in the home environment.
*Implementing interventions in the patients' health and social needs which would assist in alleviating anxieties being experienced
*Participate in Team meetings pertaining to the Trust and the First Response and Recovery and Well Being Service.
*assist in the co-ordination of various social recovery groups run by the service
*Maintain accurate and up to date records of patient care with due regard to confidentiality of patient information
*Responsible for updating own knowledge of contemporary psychiatry with particular relevance to Adult clients suffering from severe and enduring mental health problems in the community.

Person specification:
The applicant must have a recent experience with mental health patients within the community.
The applicant must also DBS cleared and have previous experience in the field.
