Job description: Service Care Health are looking for a Band 5 Community Nurse (RGN) in Swindon.

The team has three Nursing teams covering 1. Malmesbury area, Royal Wootton Bassett and Purton.

The post holder will work Core Hours of Monday to Friday 08:30hrs to 16:30hrs, however part time applicants will also be considered.

Main responsibilities and duties:

1.Provide support and education to peers, new staff non-registered staff and students.
2.To work within the integrated multi-professional team.
3.To be accountable for assessing, interpreting, planning and implementing nursing treatment and care to patients within professional guidelines, with the support of specialist team members and the team leader.
4.To undertake assessments, care planning and evaluation.
5.Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.

You need to have at least 12 months NHS experience, a degree or equivalent in nursing and an NMC registration to apply for this role.
