Job description: Community Psychiatric Nurse, Band 6.

Vacancy for a community care coordinator to work in the Barnet East locality team.

Opportunity for an experienced Mental Health Nurse to provide effective care and specialist advice for people with psychosis and complex mental health needs and their carers, in order to support their well-being and recovery.

The Barnet East Locality Team provides community-based treatment and support for adults with moderate to severe mental health difficulties who live in the borough of Barnet and/or registered with a Barnet GP. The care delivered is Recovery-focused which involves a change in focus from managing problems and symptoms to focusing on an individual's strengths and goals for the future

The ideal applicant will have experience of carrying overall responsibility for the management of a caseload, acting as a care coordinator, as defined by the Care Programme Approach (CPA). Designing individual programmes of care collaboratively with patients to maximise self-management and social inclusion and reflect their own goals and aspirations. Reviewing and adjusting plans according to changing service user needs.

The successful candidate will join a small team in which there are continuous learning and development opportunities and a record of effectively supporting people with complex and long term difficulties to find a way to live their life with more satisfaction, freedom and sense of achievement.

The role is a full time NHS contract, Monday - Friday 9-5.
The hourly rate for the role is £26 p/h.
