Job description: Service Care Solutions are currently on the lookout for a Band 7 Nurse Practitioner to work within a Primary Care Health Centre in Whalley, Lancashire.

The position is full time for the next 8 weeks with the potential of extension. The position is Band 7 paying £26 - £34ph (Umbrella Ltd) Depending on time of shift worked.

The post holder, as part of a team, will provide a flexible service to respond to the physical health care needs of patients within Learning Disability Division. In conjunction with the multi-disciplinary team, the practitioner will co-ordinate all areas of physical health care and health promotion and actively support and work with hospital ward staff to meet the physical health care needs of the patients.

The post holder will be an independent practitioner demonstrating advanced skills and competency in the assessing, diagnosing and treating of patients with a variety of illnesses, injuries and ailments and chronic disease.

The post-holder will have general responsibility for educating and supporting ward staff in the delivery of physical health care.

The post holder must communicate highly complex information, which may be highly sensitive regarding the Health care of the patients in a hostile, highly emotional environment, where there are barriers to understanding due to mental health issues in a specialist Learning disability Hospital

Examples:
1.Counselling patients prior to testing for sexually transmitted disease.
2.Informing patients of results of sexually transmitted infection screening, which may be positive for hepatitis ABC, HIV, Chlamydia, syphilis and gonorrhoea.
3.Counselling patients regarding sexual practice (particularly if this is associated with the patients index offence i.e. rapists) this can be highly emotive and antagonistic.
4.Discussing lifestyle changes regarding these practices.
5.Highly skilled communication is required to bring about behavioural changes in this client group and to persuade the patient to choose treatment options to improve their physical health.
6.Verbal de-escalation skills are required in these highly emotive and antagonistic consultations.
7.Written communication and electronic communication are essential responsibilities of the post holders to communicate this information to the multi-disciplinary team.
8.The unpredictable nature of both the clients and the daily work load requires that the post holders have the ability and skills to deal with constantly changing and unpredictable demands.
9.The post holders are expected to promote the service at national level e.g. Presentations at national conferences.

Negotiating and influencing skills are required. Examples:
1.To put forward the nursing view point to the multi-disciplinary team.
2.Teaching skills to teach and develop staff
3.Support and appraisal skills essential to manage staff in a stressful, challenging and changing environment.
4.The post holders must be expert practitioners who can inform, educate and influence both patients and staff.

The successful application must:
Be an NMC Registered Nurse with previous experience within Primary Health Care supporting a range of Mental Health and Learning Disability Service users.
Have previous Band 7 experience clearly documented in your CV

It would be beneficial if the applicant holds a current online transferable DBS and have full evidence of Hep B, MMR and BCG vaccinations.
