Vacancy in the CAMHS Learning Disability team in Barnet.

The North Central London Transforming Care Partnership is seeking a highly skilled and experienced person to support CAMHS commissioners to deliver robust support planning for children and young people who are at risk of admission by carrying out community Care and Education Treatment Reviews (CETRs) and promoting multi-agency working for individuals with the most complex needs. The Care Coordinator may be a qualified RMN, Learning Disability Nurse or Social Worker with expertise in CAMHS, learning disabilities and/or autism, or have relevant commissioning experience within health or social care settings.

The main objectives of these temporary posts is to co-ordinate and manage the work of the Enfield Transforming Care Enhanced Family Support Service (ETCEFS Service) and also facilitate improved multi-agency working so that children and young people receive appropriate support, information, advice, signposting, and professional expertise as and when required in order to remain well in the community. There will also be an expectation that admissions to residential or CAMHS inpatient units will be prevented through quality risk management processes, including the completion of community and Blue Light CETRs as required.

Candidates must be able to demonstrate the ability to manage their own caseload, and to be able to produce complex care plans that can be used to aid patient recovery. The ability to work autonomously and under pressure is essential.

The role is a full-time NHS role, on a 12 month contract, working Monday-Friday 9-5.
The hourly rate is 31 p/h ltd
