Job description: REMOTE WORKING CHC NURSE ROLE AVAILABLE! An exciting opportunity has just become available for a Band 6 or Band 7 Learning Disability CHC Nurse Reviewer to work remotely for the NHS!

Temporary pay rate
*Band 6 Standard Days: £26ph ltd
*Band 7 Standard Days: £30ph ltd

Hours and length of contract:
*Full time 37.5 hours
*Monday to Friday 09:00 - 17:00
*The manager is looking for the position to be filled ASAP with an initial 3 months contract.

Daily Duties
As a Band 6 CHC Nurse Assessor you will be a registered nurse and be responsible for determining the eligibility for NHS Continuing Healthcare funding through new assessments and reviews. You will independently manage a case load Learning Disability service users to review their eligibility for NHS CHC funded care in accordance with National Framework for NHS Continuing Health Care & Funded Nursing Care. The successful candidate will be able to recognise and implement safeguarding process and act within the principles of the MCA.

Daily duties include:
*Providing assurance that the current NHS CHC funded care is meeting the assessed primary health need in accordance with National Framework for NHS Continuing Health Care & Funded Nursing Care
*Undertake reviews of service users, where quality and safety concerns have been identified and complete as required project reporting in accordance with safeguarding and local process.
*Ensure care is safe, least restrictive and the principles of MCA is implemented in accordance with National Framework and relevant local DoLS/ CoP process
*Actively promote communication and partnership working between the statutory, independent and voluntary sectors.
*Communicate relevant information and to provide advice and support on continuing health care to service users and carers
*Work collaboratively with CSU in ensuring that the assessed primary health need is commissioned and brokered in accordance with National Framework for NHS Continuing Health Care and Funded Nursing Care, LD Transforming Care Framework & Valuing People
*Promote Personal Health Budgets and local funding pathways
*Attend meetings as required within the CLDT and or CCG/CSU
*Liaise with other health professionals on clinical matters as part of the review process
*Maintain professional confidentiality in all matters relating to patient care

Person specification:
The applicant must be DBS cleared and have previous experience with National Framework for NHS Continuing Health Care.
