Job description: Service Care Solutions are on the lookout for experienced Band 7 Case Managers / Care Coordinator to work within an NHS Community Mental Health Service.

The successful applicant will join a Multi-Disciplinary Team (MDT) of professionals in the Manchester Locality, to support Adults with complex care needs, and manage a caseload of identified patients who have complex mental health and social care needs.

The post holder will have responsibility for proactively monitoring, and managing patients on the caseload in a timely manner; as well as co-ordinating care and navigating the patient through various pathways.

The caseload will include patients across the full range of community settings. You will be part of a small core team of staff whose remit is to undertake work specifically around admission avoidance and or supported hospital discharge, developing links to the larger support mechanisms of community services. The post holder will manage the recruitment and retention of a caseload of patients with specific health needs. You will provide programmes of care which increase patient's knowledge in respect of their condition and promotes self-management.

The manager requires a Band 7 Practitioner; Registered Nurse, Qualified Social Worker or Occupational Therapist with recent clinical experience within a similar position. The applicant must be fully registered with the relevant professional body; Nurses with the NMC and QSW or OT with the HCPC. Transferable DBS would be desirable for the post. The ability to commute within the organisations geographical locality with access to an appropriate means of transport is essential.

The working hours are 37.5 hours per week consisting of 5 shifts of 7.5 hours; 09:00 - 17:00 Monday to Friday. The post is Band 7 paying ££27p/hr (Ltd). The post is initially for 8 weeks / 2 months with the potential of extension.
