I am looking for a Community Psychiatric Nurse for a role with an Early An adult Community Mental Health Team in Bradford. The role is Band 5, full time and will involve working, 09:00-17:00, Monday-Friday for a period of 3 months although this may be extended.

Salary - Band 6

£18.00(Umbrella Limited) per hour, depending on experience.

Role Summary:

Community Mental Health Nurse will provide community based care to clients with complex mental health needs who have been referred into the integrated Community Mental Health Team. They will work as an autonomous practitioner managing and prioritising individual caseload and will act as a Care Co-ordinator within the Care Programme Approach. The post holder will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for a defined caseload of people with complex mental health and/or social care needs.

The post holder will provide high standards of skilled mental health nursing care to individuals and their families within a defined geographical area. The role will include active participation in the service planning and development of the nursing and multi-disciplinary teams.

The post holder will provide clinical supervision and support to junior members of the team and actively promoting the values and policies of the service and the Trust. The Community Mental Health Nurse will be an active member of the multi disciplinary team and will work in close co-operation with secondary and primary care based professionals, other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies.

-Registered with the NMC as a Registered Mental Health Nurse
-Nursing qualification
-Recent experience working in the community
-Must be able to engage with patients/clients
-Must be a car driver.
-Evidence of vaccination/immunity for TB (BCG), Hep B, MMR and Varicella (chicken pox).
-Must be eligible to work in the UK
