Job description: Cardiff Council are looking for an experienced Adults Social Worker to join a busy community mental health team for older adults based at Llandough Hospital. This is a full time, temporary position being offered for an initial period of 3 months and with an immediate start available!

As part of this role you will be completing assessments and amending care plans to meet assessed needs/outcomes. Examples might be arranging respite in a nursing home, increasing care plans due to changing need, arranging mediation meetings between service users and agencies, completing Mental Capacity Test and Best Interest decisions. The ideal candidate will have experience of working with Older People and good knowledge of mental illness, Mental Capacity Act and Mental Health Act.

Interested candidates will need to have:
*Social work degree, or equivalent
*HCPC or Care Council Wales registered
*Experience of working with adults and completing assessments
*Experience of working in a mental health team
*Full UK driving license and vehicle
