Job description: Our NHS client requires a Band 5 community nurse to join their community integrated care team, delivering care to patients in their own homes across various locations in Leeds. You will join a team made up of community nurses, matrons and therapists. Whilst working closely to improve or maintain the health of mostly housebound people and encourage them to become more independent, you and the teams will also work with other health and social care providers and third sector organisations in the area.

Areas of care provided by the Integrated Community Care Teams include:

- Co-coordinating and managing the care provided to patients with complex health needs
- Providing support and advice to patients in relation to ongoing treatments or following surgery
- Wound management including leg ulcers
- Health promotion for individuals, carers and communities
- Assessment for equipment

Your daily responsibilities will include but are not limited to:

- Providing and sustaining high standards of individual patient care in accordance with NMC guidelines
- Analysing and Implementing care plans that fulfil patient needs
- To implement the planned programme of nursing care and, where appropriate, liaise with and co-ordinate other members of the caring team who may be responsible for implementing aspects of care.
- Organise and manage own time, delegating work appropriately.
A passion for providing high quality care, a positive attitude and good communication skills are also desirable.

To be considered for this role, you must hold a Degree in Adult Nursing or equivalent, have up-to-date registration with the NMC, have at least one years NHS Community experience and have access to your own vehicle.
