Job description: Service Care Health are currently seeking experienced and enthusiastic Band 5 Community Nurses to join a number of teams delivering care to patients across Sheffield. Although working as part of a team, you will predominantly be working alone, managing your own caseload of patients.

To be considered for this role, you must hold a Degree in Adult Nursing or equivalent, have up-to-date registration with the NMC and have access to your own vehicle.
Your daily responsibilities will include but are not limited to:

- Providing and sustaining high standards of individual patient care in accordance with NMC guidelines
- Analysing and Implementing care plans that fulfil patient needs
- To implement the planned programme of nursing care and, where appropriate, liaise with and co-ordinate other members of the caring team who may be responsible for implementing aspects of care.
- Organise and manage own time, delegating work appropriately.
A passion for providing high quality care, a positive attitude and good communication skills are also desirable.
