Job description: I am currently recruiting Band 6 Community Mental Health Practitioners to join an NHS based Adult Home Based Treatment Team (HBTT) in Greater Manchester working nights.

Hours: 37.5 hours per week, 21:00pm-8:00am - 3 shifts one week. 4 shifts the week after.
Contract: 8 weeks (+ongoing)

Salary: £23.00p/hr (Ltd) Days £28.00p/hr (Ltd) Nights and Saturdays £33.00p/hr (Ltd) Sundays and Bank Holidays

The team supports people who are currently in crisis or having a relapse of an existing mental illness and require intense support. They will provide a rapid assessment and treatment in people's homes or community venues, to avoid inpatient admission.

The team work closely with wards to facilitate discharge from hospital by increasing support at home in the initial period following inpatient discharge.

The team is a 365 days a year service and covers the 24-hour period and access to a qualified health professional is available 24 hours a day.

HBTT provide clinical and therapeutic interventions; 1:1 support, medication monitoring, daily planning and activities, anxiety management, risk management and safety planning and much more.

Person Specification: The successful applicant must be a Qualified Mental Health Nurse who is currently registered with the NMC or a Qualified Social Worker currently registered with the HCPC. The applicant must have a minimum 6 months employed experience working in Community Mental Health in areas such as CMHT, Assertive Outreach, Early Intervention, Crisis Resolution, Primary Care etc. The applicant must hold a full valid driving license and a car is essential to use as part of the job role.
