Job description: We are looking for Band 6 Community Mental Health Nurses or Mental Health Social Worker (CPN) for a fantastic opportunity to join a specialist Mental Health service based in Suffolk, within the NHS.

Applicant must drive and have access to a car.

Temporary pay rate: £24ph

Hours and length of contract:
*Monday - Friday 09:00-17:00
*Initial 3 month contract (likelihood of extension)
*The manager requires the position to be filled ASAP

Daily Duties:
As part of the Enhanced Community Team (ECT) you will support working age adults from 25 to 70 years old with complex mental health needs within the community to provide high quality, evidence-based, holistic nursing care.

Conditions include:
*personality disorders
*depression or anxiety
*and stable psychosis
You will be responsible for care coordination of a caseload which will involve supporting the service users under Care Program Approach (CPA) pathway and no CPA pathway. You will be responsible for assessment and monitoring of mental state of the service users, providing psychoeducation, monitoring therapeutic effects and side effects of medication, signposting the service users to different services and making referrals to housing services and benefit services.

You will be working as part of the multidisciplinary team which includes psychiatrists, psychologists, mental health nurses, social workers, an occupational therapist, assistant practitioners and support workers.

Person specification:
The applicant must be DBS cleared and have previous experience in the field.
