Job description: Delayed Transfer of Care Delivery Programme Lead

We are currently recruiting for 2 X Delayed Transfer of Care Delivery Programme Lead to work in the London Borough of Hammersmith and Fulham.

Working Hours: 36, full time
Start Date: ASAP
Contract Length: Contracted until 30th March 2018
Pay rate: Depending upon experience, Negotiable

About you:
*You will be skilled in programme design and delivery.
*You will also thrive on the opportunity to lead the delivery of a major and complex change agenda and be comfortable in managing the different demands and aspirations of a range of diverse stakeholders.
*You will have a track record in delivering improvements in operational processes, experience in working in acute sector or local authority is beneficial particularly in relation to the delivery of managing transfers of care service change

About the role:
*The London boroughs of Westminster, Kensington and Chelsea and Hammersmith & Fulham, Central London, West London and Hammersmith and Fulham CCGs are working together to review the system for patient discharges that are complex and have multiple needs.
*The CCGs and LA's have recently had the BCF plan assured by NHSE.
*We are now embarking on a range of programmes to support service transformation in areas covered by the Better Care Fund.
*A requirement of the BCF submission was a joint action / improvement plan to develop and implement policies and processes to reduce delays in discharge when patients are medically fit.
*We currently work with 2 acute providers - one of the roles will work directly with Imperial Healthcare NHS trust and system partners, the other Chelsea and Westminster and system partners.
*These are roles where there is an immediate opportunity to make an impact and to work with the systems when they are at their busiest over the winter period.
