District Nurse - St. Helens

Job description: Service Care Health are currently seeking experienced and enthusiastic Community Nurses to join a number of teams delivering care to patients in St Helens.

Although working as part of a team, you will predominantly be working alone, managing your own caseload of patients.

You must hold a Degree in Nursing or equivalent and have up to date registration with NMC.

Your responsibilities will include but are not limited to:
*To promote and deliver the highest standard evidence based patient care, utilising resources available to the benefit of the patient.
*To be responsible for the initial assessment of care needs, the development, implementation and evaluation of programmes of care.
*To implement the planned programme of nursing care and, where appropriate, liaise with and co-ordinate other members of the caring team who may be responsible for implementing aspects of care.
*To recognise situations that may be detrimental to the health and well-being of the individual.
*To be responsible for maintaining accurate records in accordance with the NMC Standards for record keeping.

These roles are available to start as soon as possible on a Full- time, 3-Month contract basis initially, with a view to extension.
