Duty and transition nurse, Band 6.

Vacancies working within the duty hub of the Recovery Team, treating patients in Hounslow.

A Duty and Transition nurse is needed to join a busy team, treating mental health patients. The role involves assessing and supporting the patients. The ideal candidate will be fully responsible for carrying out full clinical and risk assessment and be sensitive to their patient's needs whilst reporting the risk to senior staff where necessary. High quality care and ensuring standards are the highest possible, are crucial.
The post holder must also respond to crisis calls; liaise with Single Point of Access/Crisis and Assessment Team, CAMHs, GPs and Primary Care.

The candidate will work at all times to promote the safety and the wellbeing of service users and their families/carers. The post holder will assess, plan and implement care and provide specialist nursing advice whilst carrying out specialist nursing procedures.

Candidates must be able to demonstrate the ability to produce complex care plans that can be used to aid patient recovery. The ability to work autonomously and under pressure is essential. Experience working in a triage role would be beneficial.

The role is to commence ASAP and the role is full time, working four days a week, 37.5 hours.

Candidates must be a qualified RMN with a valid NMC Pin.

The hourly rate is £24.50 p/h
