Job description: The role is a full time position that operates on a 3 month ongoing contract with a strong possibility of extension and is able to offer flexible working hours. The successful candidate will be providing support to young people aged 18-25 within the leaving care framework.

You will be providing a targeted, outcome focused service to children and young people who are legally defined as a care leaver and therefore entitled to services. You will also be involved in assessment processes and support interventions will be carried out based on their pathway plan, as well as ongoing support, advice and guidance to develop the skills necessary to live independently.

Ideally you will need to have:
* Experience of working with you children and young people
* Previous experience of working with challenging groups
* Statutory and legislative knowledge in regards to the transition to adulthood
