Job description: Health & Wellbeing Coordinator, Liverpool
A national housing association are recruiting for a Health & Wellbeing Coordinator in Liverpool. The role will facilitate the coordination and delivery of a range of community and scheme based wellbeing opportunities including social, leisure, health, wellbeing and recreational activities for customers in partnership with local community groups, services and volunteers across a geographical region of Retirement Living Services working as part of a team committed to 'best practice' in the social housing field.

The Role
The focus of the role is to maximise the use of scheme facilities with promotional activities to encourage participation from the wider community and deliver initiatives that promote independence and encourage healthy, active lifestyles as well as address social isolation and exclusion within the customer base.
This will include the following responsibilities:
*Research and record local community groups and local health services operating in the regional areas.
*Develop and maintain partnerships and working relationships with the appropriate groups.
*Source and promote existing and emerging information about local resources and services to customers in their locality.
*Ensure the delivery of activities and programmes reflective of different customer needs, including culture and diversity.
*The coordination and delivery of health and wellbeing initiatives that promote a healthy and meaningful lifestyle for older people.
*Develop a programme of activities and events within the scheme and off site. Arrange, and manage scheme events and activities.
*Organise specific sessions in relation to energy usage, enable customers to understand the controls within apartments and the benefits advice available.
*Update the events programme on a quarterly basis by obtaining feedback from customers, participants and events promotors.
*Liaise with appropriate outside agencies.
*Create and develop an evaluation tool of the health and wellbeing service.
*Enhance the intellectual and social wellbeing of our customers enabling them to maintain their independence and enable them to lead full and active lives.
*Provide customers with a happy and stimulating experience helping them to socialise within schemes and the local community.
*Promote and encourage a high level of customer involvement, consultation and communication.
*Maintain appropriate records as required and ensure that information is organised and evaluated.
*Key performance indicators; the delivery of health initiatives on a regular basis across the region; Assist in the production and facilitation of an annual events programme; Reduced Hospital Admissions; Positive Customer outcomes.

The Candidate
To be consider for this role, it is essential that you have experience in the following areas:
*Delivering wellbeing initiatives in either housing or social care
*Working within the social care sector
*Organising events
*Supporting older people within a community setting
*Experience of using clear recording mechanisms.
*Demonstrate IT Skills to include a basic proficiency in Microsoft Office including Word and Excel as well as a willingness to learn specific IT.

The Contract
This is an initial 3 month contract, replacing a permanent leaver, so has a chance of becoming permanent. The role is full time, working 37.5 hours between 9:00 and 17:00.
