Job description: Service Care Solutions have an excellent opportunity to work for one of our newest clients in the housing sector. If you are looking to work for a large, well-structured organisation who have major growth plans in the pipeline then this is the role for you! They have open positions for two Healthy Homes officers to work on a full time FTC for the next 9 Months.
The purpose of these posts is to work alongside housing providers and a range of other partners and community organisations in private sector housing by providing visits, assessments and interventions to older people in their homes which improve their health and well-being, enable independent living and prevent avoidable health service presentations and hospital admissions.
Responsibilities include -

*To carry out full needs assessments with customers identifying any issues and agreed interventions required to enable the customer to live independently in their home and improve their health and well-being.
*To work with customers, their families and carers and partners to carry out risk assessments (including safeguarding), personal and mental health, self-care and medication management, health and safety, fire safety, security, support, life skills and financial checks and assessments health and well-being.
*Advise customers and arrange for support about the financial aspects of their housing and care and support options. This will include entitlement to welfare benefits, budgeting and debt issues, access to independent financial advice, solutions and service to help combat fuel, food and furniture poverty and access to local and community care and support services.
*Advise customers and arrange for support about possible sources of assistance which might help them to improve, repair and maintain or adapt their home, including the availability of grants, loans and equity release for homeowners. This may include referrals to a specialist services and agencies.

Essential Criteria -
*Knowledge of a housing related issues including welfare benefits.
*Experience of working with vulnerable customers and/or housing issues and how these affect customers.
*Experience of working successfully as part of a team and providing front line customer services.
*Experience of assessing the needs and risks of customers, and offer appropriate advice
*Experience of assessing personal financial circumstances, maximising incomes and benefits and developing realistic personal budget and debt management plans.
*Experience of welfare benefits and tax credit system.
*Driving License and Access to own Vehicle
*Enhanced DBS
