Job description: One of the largest housing associations in the UK are currently recruiting for a Homeless Outreach Support Worker to work with the local council to provide an assertive outreach service, to reduce homelessness across the Guildford area.

The Role
The focus of this Homeless Outreach Support Worker role is to provide an assertive rough sleeper outreach and housing related support service to clients who are homeless and are in need of temporary accommodation.
This will include the following tasks and responsibilities
*Ensuring the delivery of individual personalised support packages, reflective of different client needs, including culture and diversity issues.
*Supporting clients with day-to-day support and tenancy needs, liaising with landlords to find suitable accommodation.
*Undertaking referrals and assessments of potential clients, working with them to identify the most appropriate housing solution for their needs. Ensuring clients have access to relevant external support services.
*Meeting clearly defined client outcome targets through key-working, support plans, risk assessments and other interventions.
*Ensuring that opportunities for education, training and employment opportunities are integral to the support service. The aim being to enable clients to develop the life skills to live as independently as possible.
*Providing practical support and information to clients when entering the service and throughout their involvement with the service, e.g., assisting clients in the completion of welfare benefit claims.
*Promote and encourage a high level of client involvement, consultation and communication.
*Maintaining an accurate and timely record of all activities including the maintenance of a support planning database.
*Working in line with health and safety, lone working procedures and safe guarding requirements to ensure the overall safety and security of the clients.
*To ensure intelligence is shared with appropriate agencies regarding rough sleepers and begging in the town centre

The Candidate
It is essential that any applicant for this Homeless Outreach Support Worker role have strong previous experience of working with rough sleepers, or those at risk of homelessness and have excellent knowledge of the housing benefits system, support plans and the obstacles preventing vulnerable individuals attaining a tenancy.
As this Homeless Outreach Support Worker role is working with vulnerable individuals, you will require an Enhanced DBS, ideally on the update service.

The Contract
This is a temporary role on an initial 12-week contract. The role is full time, working on a rota between 8am - 6pm, Mon - Fri and may include occasional weekends on an on call basis.
