Job description: Hospital Social Worker, Southwark

Intermediate Care Southwark (ICS) and Southwark Hospital Discharge Team (HDT) are recruiting 2 locum Social Workers to work in the fast paced Hospital Discharge and Urgent Response/Reablement service areas.
Posts are based at Queens Road (Peckham) for ICS and Kings College Hospital for HDT.

The vacancy is full time (36 hours a week) and contracted until 28/02/21 with a chance of extension.

Note: Both posts require physical attendance at the office in Southwark although a degree of working from home can be arranged.

To apply for the role you will need:
*Proven experience in either or both of those settings or transferable skills and experience in a similar multidisciplinary health and social care setting.
*Social Qualification and registration
*Flexibility to work across both teams desirable depending on winter pressures; preference can be expressed at interview for HDT or ICS post in particular.

About the role:
*Role includes assessment of (complex) social care needs and provision of support for vulnerable adults to ensure they are safe and have good outcomes to promote independence.
*Accountable for working within the statutory legal framework, policy and guidance and determining when the threshold for statutory intervention has been reached in line with legislative and organisational standards within the Care Act 2014 (e.g. MCA/DoLS/CHC assessments etc).
*Responsible for managing your own caseload and prioritising urgent needs, maintaining accurate and timely data, attending virtual funding Panel presentations as required to secure funding for proposed care/placement.
*Reablement Goal Setting experience desirable.
*You would be planning responses to safeguard vulnerable adults concerns, including initial risk management and safety plan as required; hospital role would include enquiry duties.
