Job description: A large, national charity is recruiting for an Independent Living Support Worker in Merseyside, to provide a time limited practical and emotional support service to service-users; typically following a hospital stay to re-enable independent living.

The Role
The focus of this role is to provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission.
The post will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their own home and prevent hospital re-admission.
In the position, you will have the following tasks and responsibilities:
*Liaising with health, housing and other professionals to ensure that the needs of the individual are consistently met.
*To ensure as safe as possible a living environment for service users through carrying out risk assessments and providing information/advice whilst respecting their individual dignity, choice and rights.
*To carry out follow-up welfare checks and visits as required and assist service users with their activities to re-enable their daily living skills for up to the maximum support period.
*Provide assistance to a person returning from hospital or medical facility and those identified by local health professionals as being at risk of a further decline in health or without a support mechanism.
*Take/accompany service user to attend hospital appointments or other activities as required in the care/support plan
*To have the ability to provide on-going assessment of service users' needs, to develop a support plan to meet these needs and provide appropriate support to achieve positive outcomes.
*Recording and reporting of activities undertaken and highlighting any changes in a service user's condition and ensure that appropriate actions are taken if a service user's health deteriorates.
*Carry out needs and risk assessments with service users, liaising with referrers as necessary.
*Provide practical and emotional support such as; mobility aids loans, transport, companionship, assistance with shopping, helping access to groups and activities
*Signpost to further support available; liaising with Community Connectors, external organisations, partners and voluntary and community sector colleagues

The Candidate
To be considered for this role, you must be over 21 with a full Manual Driving License, and a full, clean Enhanced DBS. A driving license must have been held for a minimum of 2 years. You will also require the following skills and experience:
*Knowledge of health and social care, e. g. how to transfer the patient safely, how to help the patient to sit on a chair
*Knowledge of working with vulnerable adults, people with mobility issues and people with physical disabilities
*Awareness of moving and handling
*Understanding of what signs of risk at home to look for and how to ask if any are found
*Manual driving licence & own vehicle

The Location
The role will be based at one of the following locations:
*Rainhill, Prescot L35 5DR
*Saint Helens WA9 3DA

The Contract
This is a full time position, working 35 hours per week - 10:00-17:30 Monday to Friday. The role is on a 3 month contract which has a strong chance of being extended. The role will be paying £8.72 per hour.
