Job description: Adult Learning Disability Nurse, Chatham, Band 7.

Vacancy for a LD nurse to work in the community in the Chatham area of Medway.

A senior LD nurse is needed to provide individual assessment, identified clinical interventions and direct care to service users with learning disabilities. They will also undertake management tasks, deputising for the Team manager as required, and will take a lead on mentoring and supervising others within their service area. The ideal candidate will be fully responsible for carrying out full clinical and risk assessment and be sensitive to their patient's needs whilst reporting the risk to senior staff where necessary.

The candidate will work with patients about their Learning disabilities and will hold a caseload of adults with LD needs and will take responsibility for providing evidence based assessments and therapeutic treatment programmes for these individuals.

The candidate will have a proven ability to liaise with and work in collaboration with other relevant agencies according to the needs of the adult.

Candidates must be a qualified LD nurse with a valid NMC Pin. Experience working in the community is required and confidence/competence in risk assessing and record keeping is essential.

High quality care and ensuring standards are the highest possible, are crucial.

The role is full-time, working Monday-Friday.
The role is an initial 3 month NHS contract with likelihood for extension.
The hourly rate is £29 p/h ltd.

Candidates must be able to demonstrate the ability to manage their own caseload, and to be able to produce complex care plans that can be used to aid patient recovery. The ability to work autonomously and under pressure is essential.
