Job description: We are looking to recruit an After Care Worker / Leaving Care Advisor to join Thurrock Council on a long term basis.

An Aftercare worker/ Personal advisor is required in the Aftercare team to work with care leavers and support them with independent living and smooth transition to adulthood. The job responsibilities includes providing information, advice and support to care leavers to access education, training, employment, social housing, accompanying them to relevant appointments, carry out needs & pathway plans assessments and advocate on their behalf.

1. To undertake a range of practical tasks that will help the young people achieve goals in key areas of their life, such as education, employment, health and accommodation. The practical tasks will be identified by the qualified staff within the Team, who have case responsibility for the young people, and will be designed to achieve objectives in the young person's Pathway Plan. The practical tasks will include:
1.To keep in regular contact with young people and to take action to re-establish contact with young people who have ceased contact with the service.
2.To provide practical advice to young people when they request it, and to put them in contact with appropriate staff / agencies when more specialist advice is required.
3.To contribute to the identification of the needs and abilities of young people.
4.To provide sustained support to young people to enable them to access and maintain education, training and housing.
5.To help to develop and implement individual plans to ensure these meet the needs of young people.
6.To advocate on behalf of young people with their education, training, employment or housing.
7.To contribute to agencies offering assessments of individual young people's needs and abilities.
8.To maintain records to allow the progress of individual young people to be monitored and contact maintained.
9.To enable young people to access and understand information that will help them make decisions.
