Job description: Service Care Solutions are looking for dynamic and motivated Band 6 Nurse Practitioners to work in Northampton. You will be based in the physical healthcare team within a well renowned mental health service. The working hours are Monday to Friday with a mix of early and late shifts. The service is looking for someone to start immediately.

A list of clinical duties and responsibilities are below:
*To provide direct access to patients seeking primary health care and physical healthcare and to act as an advocate for patients regarding their physical healthcare.
*To act as a resource for clinical teams on all aspects of physical healthcare.
*To provide physical health care and interventions for patients under direction of and in collaboration with Advanced Nurse Practitioner. To make clinical decisions within the boundaries of expertise and training

*Provide appropriate advice and treatments through evidence based standards of care.
*Provide clinical care in line with agreed policy, local and national protocols or guidance.
*Be able to identify medical emergencies and take the appropriate action.
*To be able to perform clinical nursing assessments/skills/interventions within the Scope of Professional Practice and safely use equipment to aid physical examinations, including (this is not an exhaustive list of skills):
*Cervical screening
*Long Term Condition / Chronic disease management
*Wound Care / Tissue Viability
*Immunisation and Vaccination
*Venepuncture
*Ear Care
*ECG interpretation.
*Continence advice and urinary catheterisation, (male and female including suprapubic)
*Pressure ulcer assessment and management
*Intravenous and subcutaneous therapy
*Skin Closure (suturing/ skin adhesive)
*Provide sexual health advice including the range of contraception for women.
*Manage and support development of treatment plans for patients with, and at risk of developing, long term conditions.
*Record clear and contemporaneous consultation notes to agreed standards using the electronic patient record systems for all patient information.
*Read code appropriately and collect data for audit purposes as agreed using Electronic Patient Record System.
*Implement and support national schedule vaccination and immunisation programmes.
*Dissemination of physical health information to patients in partnership with IPU's.
*Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
*Safely manage hazardous biological substances including blood, vomit, urine, faeces and other body fluids as per policy.
*Be willing to undertake non-medical prescribe training relevant to skills and expertise. Prescribe in accordance with evidence based practice, formulary and national and practice protocols and within the Scope of Professional Practice and legislative framework. Work with patients in order to support compliance with and adherence to prescribed treatments.
*Provide information and advice on prescribed or over the counter medication on medication regimes, side-effects and interactions.
*Encourage health promotion, education and self-care for all patients and advise them of other available services.
