Job description: Occupational Therapist, Band 6.

Vacancy working within the Community Home Treatment Team, based in Harrow, treating older aged adults.

We are looking for an experienced OT to provide high quality services to older people treating for a range of problems related to elderly care, falls, orthopedic and long term conditions.

The candidate will be carrying out OT and generic mental health assessments. Good communication skills, both verbal and written are crucial. The role will also involve preparing assessment reports and care plans as required, completing risk assessments and agreeing on management plans.

The candidate will join a busy team with the goal of preventing unnecessary admission to hospital and to promote functional independence by facilitating early discharge from hospital.

High quality care and ensuring standards are the highest possible, are crucial.

Candidates must be a HCPC registered, possess a current driving license and have access to a car for work purposes. Previous experience working in a community setting would be strongly beneficial.

The role is an NHS contract, on an ongoing basis (minimum 4 months), working Monday - Friday 09:00 - 17:00.

The hourly rate for this role is £26 p/h.

Candidates must be able to demonstrate the ability to manage their own caseload, and to be able to produce care plans that can be used to aid patient recovery. The ability to work autonomously is essential.
