Job description: Service Care Health are looking for an Occupational Therapist to work on Conwy, Wales.

The ideal candidate will be required to start as soon as possible, initially for 3 months and must be willing to work on 37.5 hour per week basis.

Purpose of the job:
-Deliver a direct Occupational Therapy service to people with disabilities
-Work with parents / carer's / families to provide support and develop resilience
-Complete screening assessments and make initial recommendations for appropriate response to need
-Complete assessments and outcome based care plans focused on early intervention, prevention and progression across the lifespan.
-Where necessary carry out safeguarding activity
-Ensure people with Disabilities and their parents / carer's and families are at the center of decision making about their lives and the outcomes that they want to achieve

To apply for this role you must have 12 months previous NHS experience, HCPC registration and a degree or equivalent in Occupational Therapy.
