Job description: We are looking for a Occupational Therapist to work with Herefordshire Council for a 4 month period, you will be contracted to 37 hours per week between 0900-1730.

Main Responsibilities:

-To manage a caseload of clients with complex needs, using evidence based client centered principles to assess, plan, implement and evaluation interventions in primary, community and secondary care settings, that maximize long-term independence and minimize the need for on-going support.

-To work as part of a Multi-Disciplinary Team, provide advice, support and training to other Team Members as required, and responsible for allocation or transfer of work to other team members as appropriate.

-Communicate complex information across and between organisations to achieve successful outcomes for the individual, and work jointly with wider MDT colleagues, organisations and agencies to ensure co-ordinated & integrated support pathways for clients are achieved.

-To consider appropriate risk management balancing the need to ensure the Service User is able to have choice and control in accordance with any risk that this may present to the Service User and others.

-To ensure current, evidenced based and cost effective practice is promoted within the team and in Directorate policy development.

-To provide supervision for identified staff in accordance with Organisational and Professional policies.

-To participate in rota systems as required to meet service need, including rotas covering a 7 day a week working pattern when required.

-All service delivery and provision to be within the framework of current legislative, Professional and Council policies & procedures.
