Job description: You will be assessing and treating community patients in their homes across the south and central regions of Oxfordshire.

This role is available on a full time basis on a 2 month contract, however a 30 hour week will be considered.

-Being proactive in picking up the patients being discharged from the JR Short stay wards to facilitate discharge and to fill any other time by working in Abingdon, Community Therapy Service, South West local team.
-Completing home assessments
-Assessing physical needs
-Prescribing exercises and equipment
-Giving advice and referring to community services as appropriate.

Essential criteria:
-Must be a driver with access to a vehicle
-Good communication skills
-Ability to work alone and make clinical decisions
-Good organisation skills
-Experience of working in a community setting
-Experience of working with older people
-Will need to be able to use own phone
