Job description: We are looking for a Physiotherapist to work with a range of physical and psychological conditions within the context of a community-based, multi-disciplinary team. This position is based in the BS34 area of Bristol.

-Diploma/Degree in Physiotherapy.
-HCPC registered
-Community experience
-37.5 hours ideally 8:30-16:30 (however start finish times are open to discussion)

Job description
-Based in a Clinic but visiting patients within the South Gloucestershire area
-Provide domiciliary physiotherapy assessments and interventions for planned patients off urgent and routine physiotherapy waiting list
-Carry out physiotherapeutic assessment, diagnosis and treatment with patients who may have diverse presentations and a range of physical and psychological conditions within the context of a community-based, multi-disciplinary team.
-To hold responsibility for own case load working with supervision as appropriate.
-Provide an evidence-based Physiotherapy service of a high standard, to patients in a community setting.
-Undertake comprehensive assessments using clinical reasoning skills and manual assessment techniques.
-Formulate an accurate prognosis and recommend the best course of intervention, developing comprehensive discharge plans from the service.
-Formulate and deliver individual Physiotherapy treatment programmes based on a sound knowledge of evidence-based practice and intervention options.
-Allocate, supervise and direct on-going Physiotherapy programmes to patients provided by support workers as required.
-Visiting patients in their own homes or in Care Homes within the Severnvale Locality of South Gloucestershire.
-The aim of the service is to maximise and maintain independence through the provision of rehabilitation
-Must be a driver with access to a vehicle, business use insurance. Must be willing to travel to and between patient visits
-Essential to have experience of working in the community
-Ideally have experience of using EMIS
