Job description: We are looking for a Band 6 Physiotherapist to work with Birmingham Community Healthcare, this will be on a full time block booking basis.

Start date: ASAP
Location: Birmingham, B7
Hours - 37.5 hours per week, over 5 days - 08:30-16:30

List of duties:

*To provide a specialist physiotherapy service to patients recently diagnosed with a stroke in a range of community settings using highly developed specialist practice underpinned by theoretical knowledge.
*To be a specialist resource of information for health and social care staff, patients, families, carers and external agencies who may require advice and support and guidance in the management of stroke.
*To transfer individual patients' on-going care to other agencies as appropriate and ensure that the service provided is conducted in a professional, efficient and effective way.
*To meet the service specification objectives and key performance indicators as part of a multi-disciplinary team providing a high quality service.


Essential:

-Valid HCPC registration
-Relevant NHS experience
