Job description: Rough Sleeper Outreach Navigators - Medway
A Kent based housing association are recruiting for a Rough Sleeper Outreach Navigator to engage rough sleepers within the Medway area and ensure they have access to the full support network, through outreach work.

The Role
The focus of the role is to work closely with the Street Outreach Team and other agencies to provide a rapid response to reports of rough sleeping and street-based activity in the county, in a bid to prevent and eradicate homelessness.
This will be achieved through the following tasks and responsibilities:
*Carry a caseload and engage rough sleepers with the Medway area through outreach work.
*Use person-centred approach to those who are reluctant to engage with support services, this may include working closely with and sharing information with statutory agencies, ensuring all KPIs are achieved.
*Provide support related to tenancy, GPs, community engagement, etc
*Manage your own case load, whilst promoting a positive environment, ensuring all service users have an equal opportunity to succeed and that their diverse needs are met.
*Provide ongoing support to clients once housed.
*Work in partnership with other agencies to ensure services are promoted, and that successful outcomes are achieved for service users.
*Able to deliver telephone out of hours support service on a rota basis - once every 3 weeks
*Comply with the relevant regulatory framework standards in every aspect of service, ensuring all relevant paperwork regarding clients and the service delivery are maintained to an auditable standard.
*Able to identify any safeguarding issues and risks.

The Candidate
To be considered for this role you must have experience of the following:
*Experience of outreach/ rough sleeper working.
*Ability to work in a flexible and creative manner.
*Ability to build rapport with service users adapting style of approach as needed, whilst maintaining professional boundaries.
*Proven ability to deal with challenging behaviour and resolve difficult situations
*Model a solution focused attitude.
*Experience of writing reports, support plans, risk assessments, incident records and court appeals.
*Understanding of safeguarding practices.
*Experience of working within defined frameworks and complying with policies, procedures and guidelines.

The Contract
This vacancy is on a 12-month Fixed Term Contract. This is a pilot scheme so could potentially be extended.
*Salary is circa £21,300
*The role is full time, with working hours of 8.45 till 17:15 Monday to Thursday, same start Friday but finish at 4.45. However these hours are flexible to the client's needs.
*There is an out of hour call system, staff will be on a rota every three weeks. Phone contact only, no going out to clients. With a £60 additional pay for this week.
