Job description: Service Care Solutions are currently recruiting on behalf of The British Red Cross for an Independent Living Co-Ordinator.

The British Red Cross help people that are in crisis and enable vulnerable people in the UK to prepare for and withstand emergencies within their own communities. The Independent Living Service supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency.

This is a part time position working 14 hours per week (Monday and Tuesday - 10am-5:30pm) and is initially on a 3-month temporary contract and is paying £10.55 hourly.

This role is based in both Whiston Hospital and St Helens Hospital

Key Features for this role:
-You will be responsible for liaising with hospital staff and other medical professionals in the community, including GPs to make patient referrals.
-Assigning incoming referrals to a Service Assistant, monitoring the service delivering, managing the budget and overseeing record keeping.
Principal Responsibilities
-To be a single point of access for referrals from external agencies and provide daily coordination and support for volunteers and service assistants.
-Ensuring service users' needs are assessed.
-Liaise with hospital and community staff from external agencies to promote the service, receive referrals and provide a timely response to questions about the eligibility of the patient.
-Undertake service user visits if necessary.

Essential Criteria:
-Full manual driving license (2 years held)
-Over 21
-Full, clean, enhanced DBS
