Job description: Cardiff Council are looking for a Social Worker to join their growing services for children, young people and young adults with disabilities services. Adopting a signs of safety approach, your ongoing case management and risk monitoring will include undertaking of a variety of assessments and the implementation and review of care and support plans - as underpinned by the Social Services and Well-being (Wales) Act 2014. The service is co-located with targeted services case management teams within Cardiff County Hall, with agile working in place to support staff to work independently across the city with a number of touch down bases across Cardiff.

Social workers within the Child Health and Disability team work collaboratively with colleagues across a range of agencies and council directorates from Health, Education, the voluntary sector and Adult Services. These posts offer a unique opportunity to support children and young people with physical and learning needs who may require Care and Support plans, Transition Planning, Child Protection Plans, Looked After Children Procedures and Court Proceedings.

Alongside regular supervision and peer meetings you will have the opportunity to attend a variety of training courses and practice enhancement to build on your portfolio of continual professional development. In addition, newly qualified practitioners will be supported through their first year in practice (CPEL).

The ideal candidate will have:
*Social work degree, or equivalent
*HCPC or Care Council Wales registered
*Experience of child protection
*Experience of working with children and young people with disabilities
*Full UK driving license and vehicle
