Job description: We have a new post came up today for a Workers to join Surrey Council on a permanent basis. This post offers a part time 4 day week.


About the Role

This post is part of our ongoing commitment to the integration and joint working for older people with mental health needs in Surrey. We are particularly looking for a qualified practitioner who has experience of working with older people or adults with mental health needs who want to develop their knowledge and skills in this particular area of practice.
You will provide a social care service to older people living in the community and to the inpatient units within Surrey and Borders Partnership NHS Foundation Trust, offering support and advice on discharge arrangements and to community support. Your work will include being care-coordinator for a number of older people with a variety of needs arising from dementia or functional mental health conditions such as depression or psychosis.

What you need to succeed.
Successful applicants will have substantial post qualification experience of routinely working with complex cases. Because of the nature of the work it is essential that post holders have a detailed working knowledge of the statutory context of adult social provision and in particular an applied knowledge of the provisions of The Mental Capacity Act. Promoting independence, choice and control is at the heart of these posts.
A valid driver's licence and the availability of an insured vehicle for the performance of work related duties is an essential requirement of the role. Applications from those who do not meet this requirement will not be considered.
