Job description: Permanent Social Worker, Mental Health / Learning Disabilities, Wandsworth

Service Care Solutions are currently recruiting for an Experienced Mental Health / Learning Disabilities Social Worker in Wandsworth, South West London. This is a Permanent role.

The role is Full Time and to start as soon as Possible.

It is paying £45K per annum.

Essential requirements include:
*Qualification in Social Work with HCPC registration
*Previous Experience as a Social Worker with People with learning disabilities and/or complex mental health needs

Some of your duties will include:
*Ensure that all care, assessment and treatment is delivered within the clinical ethos of the unit.
*Provide comprehensive reports as required for use in clinical meetings, MHT, CPA meetings, Hospital Managers reviews and the Courts.
*Be responsible for ensuring the highest standard of clinical practice is achieved and that the social care delivered to patients is of a consistently high standard.
* Work closely with other professional staff ensuring a consistent pattern of care for each individual patient.
*Assist patients to obtain their full entitlement to benefits and to maintain a high level of knowledge and expertise in this area.
*Help to ensure that the admission, leave and discharge of patients is appropriately and sensitively organised and achieved with the minimal amount of disruption to the patient.
