Job description: Support Coordinator - Harrow
A local authority in Harrow are recruiting for a Support Coordinator to provide a needs & risk assessment service to prospective and existing Sheltered Housing Tenants across the area.

The Role
The focus of the role is to manage a case-load of vulnerable tenants, to ensure their support structure is coordinated effectively to best suit their needs. This will include the following tasks and responsibilities:
*To deal effectively with a range of complex support needs and risks that have significant implications for Tenants and prospective Tenants.
*To monitor the wellbeing of Tenants through daily or weekly contact as agreed.
*To participate in a weekly rota to provide onsite support surgeries at each scheme.
*To participate in a rota to undertake needs and risk assessments for prospective Tenants. These assessments are undertaken in applicant's homes.
*To prompt, enable and monitor Tenants to manage prescribed medication as advised in care and support plans, recording as appropriate.
*To provide time limited, structured and flexible support to Tenants.
*To develop and maintain professional working relationships with local voluntary and statutory agencies to enable Tenants to access appropriate services and support.
*To ensure that all records are accurate, legible and up to date in order to evidence outcomes for Tenants, performance targets and to comply with funding requirements.

The Candidate
To be considered for this role you will require previous experience in a support coordination role and command a strong knowledge and understanding of support delivery and welfare and benefits. You must be able to prioritise your workload and have experience of developing needs & risk assessments and support plans. Due to the nature of the clients you will be working with, an enhanced DBS is also a requirement. These can be processed during the application process, but an existing DBS on the update service will be beneficial.

The Contract
This is a temporary contract on an initial 4-week contract with possible extension. Working hours for this role is 7 hours 12 minutes daily = 36 Hours a week- Monday to Friday. Lunch break must be taken between the hours of 12:00 to 14:00.
