Job description: One of the largest housing associations in Kent are currently recruiting for an Support Worker to join their Wellbeing Services team in Medway, to work with service users towards independent living.

The Role
The focus of this Support Worker role is to work with service users with regards to their health and wellbeing, helping them to live as independently as possible.
This will include the following tasks and responsibilities
*Engage service users, providing practical support with life skills and achieving positive outcomes within agreed timescales demonstrating a high level of customer service.
*Deliver the key performance indicators of the service contract.
*Manage the case load throughput, ensuring you seek new referrals as well as closing cases appropriately in order to maximise the service delivery and the benefit to the community.
*Collaborate with colleagues to deliver the objectives of the Community Strategy Action Plan.
*Comply with the relevant regulatory framework standards in every aspect of service, ensuring all relevant paperwork regarding clients and the service delivery are maintained to an auditable standard.
*Comply with all policies and procedures to ensure the safety and well-being of yourself, your clients and your colleagues.
*Represent West Kent, working in partnership with other agencies to achieve agreed outcomes for service users.
*Promote a positive environment ensuring all service users have an equal opportunity to succeed and their diverse needs are met.
*Model living West Kent's values.
*Undertake any other duties to achieve the purpose of the job.

The Candidate
It is essential that any applicant for this Support Worker role has strong experience providing housing related support, either in the community or within a supported housing service. You will require good knowledge of support plans, carrying a case load and tenancy management.
You will also need an ability to work with challenging behaviour, be able to communicate well, including writing reports and have good knowledge of MS Office and the ability to use in-house systems quickly.
As this Support Worker role is working with vulnerable individuals, you will require an Enhanced DBS, ideally on the update service.

The Contract
This is a temporary role on an initial four month contract. The role is full time, working on a rota between 8am - 6pm, Mon - Fri and may include occasional weekends on an on call basis.
