Job description: Supported Living Coordinator - Dudley
A local authority in the West Midlands are recruiting for a Supported Living Coordinator to provide and maintain an efficient service to tenants living in Sheltered Housing Schemes across Dudley, by identifying and ensuring that their support needs are met, in order to enable a diversity of older vulnerable people to live independently in their own homes.

The Role
The focus of the role is to provide and maintain an efficient service to tenants living in Sheltered Housing Schemes by identifying and ensuring that their support needs are met, in order to enable a diversity of older vulnerable people to live independently in their own homes.
This will include the following tasks and responsibilities:
*To carry out the identified Needs Assessment for prospective tenants to ensure that their support and any other needs are clearly identified and that they meet the criteria for becoming a tenant in Sheltered Housing
*To provide a service to tenants through regular assessment and review and carry out a wellbeing assessment at least six monthly or when there is a change in either (a) the predictive risk assessment or (b) individual circumstances in accordance with the appropriate policies and procedures
*To initiate and co-ordinate packages of care and support for tenants, with the appropriate services, providing reports as required and arranging visits and services
*To assess regularly the mental, physical and emotional wellbeing of tenants by observing and listening to them, whilst giving appropriate advice and encouraging tenants to accept support, enabling them to maintain their wellbeing and independence, by using wellbeing assessments, reviews and predictive risk assessments as a tool to measure these
*To collect and document accurate personal data from tenants, ensuring that it is up to date and recorded confidentially in the appropriate systems at the Scheme in compliance with Data Protection
*To work alongside tenants and family members to encourage their involvement in planning and providing the appropriate levels of support for the tenant
*To actively encourage feedback from tenants, their families and other agencies, and to ensure that it is used positively to improve services

The Candidate
To be considered for this role, you will must be able to demonstrate experience of working in a building management or similar role with responsibility for health and safety functions and have substantial experience of providing care and/or support to older and/or vulnerable adults.
You must be able to demonstrate knowledge and use a range of ICT systems and solutions including MS Office and a willingness to undertake IT training as required. You will also hold the IOSH award or have a willingness to undertake relevant Training and ideally have a BTEC Housing Certificate level 2 or level 3 or other recognised qualification such as NVQ level 3 Promoting Independence/Health & Social Care / CQF Diploma in Health & Social Care Award or have a willingness to undertake relevant training.

The Contract
This is a full time role on an initial 3 month contract, covering the recruitment for a permanent position, which you will have the chance to apply for once in post.
