Job description: We are looking for Band 7 Community Case Load Managers for a fantastic opportunity to join a specialist Mental Health service based in Chelmsford within the NHS.

Temporary pay rate: £30ph

Hours and length of contract:
*Full time 37.5 hours
*Monday to Friday 09:00 - 17:00
*The manager is looking for the position to be filled ASAP with an initial 3 months contract.

Daily Duties:
You will be working as a Community Caseload Manager for the community pathway team who provide therapeutic interventions for service users with complex mental health needs such as: depression, anxiety, personality disorders, OCD and PTSD.

You will assist with case load management to care co-coordinators to help identify patients who are appropriate for transition into the community and make proposals to support sustainable management of resources.

You will monitor referrals, allocations, discharges and local trends and patterns around discharge as well as having responsibility for the day-to-day supervision and management of lower banded staff within your professional group and team locality.

Clinical responsibilities:
*Review progress towards discharge/transfer during case supervision
*Support staff with complex cases, facilitating MDT meetings, CPA and other forums as required.
*Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation / records on all service users in line with your Professional Code of Conduct and Trust policy
*Conduct and supervise assessment and observation to ensure appropriate and timely feedback to the team

Non - Clinical responsibilities:
*Manage and coordinate multi-disciplinary meetings
*Identify aspects of the service which need to be improved and implement appropriate changes in conjunction with the Clinical and Team Manager
*Participate with the Clinical and Team Manager for the recruitment of employees
*Deliver training where appropriate for other professionals
*Participate in SI investigations, disciplinary and grievance procedures

Person specification:
The applicant must be DBS cleared and have previous experience in the field.
