Job description: Trusted Assessor (Adult Social Care Coordinator)

Headington - Hospital Based
Pay rates up to £18.00 ltd per hour!
3 month locum contract - likely to be extended

As part of ongoing initiatives to improve user experience and reduce unnecessary hospital delays, we are recruiting to two Trusted Assessor posts in Oxfordshire initially for a period of three months.

The underlying principle of the Trusted Assessor approach is to promote safe and timely discharges from hospital to adult social care services. This approach allows adult social care providers to adopt and use assessments carried out while people are still in hospital.

*Using trusted assessors to carry out a holistic assessment of need avoids duplication and speeds up response times so that people can be discharged safely as soon as they are medically fit for discharge
*Key to success of the pilot will be based on relationships between professionals based on a shared belief that nobody should be in hospital for longer than necessary. This will include team members, Social Workers, Nurses and Therapists, pilot providers and Sourcing Team.
*Pilot Providers will need to be confident that Trusted Assessors understand the needs their service can meet. They will also need to be confident that any discharges to their service will be appropriate and meets the needs of the service user. Risks associated with the persons care needs and the transition from more intensive care available in a hospital setting to the pilot provider must be considered and addressed.
*In return you will be part of the new Winter team supporting the hospital and other services through the winter period. You will be based with the team at the John Radcliffe Hospital.
*You will be expected to work 08:30 - 17:00. Including on a rota basis at weekends.
*You will need to have experience of working in a care setting with the ability to generate good quality home care assessments including risk assessments and care plans. You will have tenacity, drive, excellent communication skills, be highly motivated alongside a strong desire to influence change. You will need to be able to engage people at all levels, and strive to achieve outcomes that create positive results.
