Candidate Responsibilities:
- Deliver evidence-based clinical services within scope of practice
- Prescribe medications according to PGD and review their effectiveness
- Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions
- Support health promotion and wellbeing programs
- Manage patients at risk of long-term conditions to prevent adverse health outcomes
- Work collaboratively within a multidisciplinary team, providing clinical supervision where needed
- Lead reviews of clinical policies and procedures
- Provide routine nursing care following NICE and NSF guidelines
- Support the development of individual care plans for chronic disease management
- Qualified Advanced Nurse Practitioner with prescribing rights
- Experience working autonomously in primary care settings
- Strong knowledge of NICE guidelines and NHS standards
- Excellent communication and patient management skills
- Ability to develop and evaluate health programs and treatment plans
