Care Navigator - Primary Care
 

Service Care Solutions are recruiting for a Care Navigator to support Primary Care services in the Yorkshire Area. This is a fantastic opportunity to help contribute to providing patients, the right care, at the right time.
Location: Yorkshire
Contract: Permanent 

Duties: 

• Supporting patients in accessing appropriate services by signposting to clinical or non-clinical support based on their needs
  
  
• Managing and triaging patient enquiries (by phone, in person, or online) and directing them to the most appropriate member of the healthcare team
  
  
• Following safeguarding protocols and escalating any concerns to clinical staff or safeguarding leads as required
  
  
• Handling sensitive information with discretion and maintaining patient confidentiality at all times
