Help Shape Brighter Futures for Young People Transitioning into AdulthoodTameside Adult Social Care is seeking an experienced and passionate Mental Health Transitions Social Worker to join our specialist team supporting young people as they move from Children's Services into Adult Services.
This is a rewarding opportunity to work with young people experiencing moderate to severe mental health difficulties, including those with autism and dual diagnoses, ensuring they receive the right support at a critical stage in their lives.
You'll play a key role in promoting independence, planning for adulthood, and helping young people and their families navigate what can often be a complex and challenging transition.
About the Role
As a Mental Health Transitions Social Worker, you will provide a comprehensive social care service to young people approaching adulthood, working collaboratively with families, education providers, health professionals, and partner agencies.

About the Role
As a Mental Health Transitions Social Worker, you will provide a comprehensive social care service to young people approaching adulthood, working collaboratively with families, education providers, health professionals, and partner agencies.
Key responsibilities include:
- Supporting young people transitioning from Children's Services into Adult Services.
- Undertaking Care Act assessments and developing clear, outcome-focused support plans.
- Completing funding applications and statutory documentation.
- Managing complex legal work including Court of Protection DoLS applications, DoLS, Section 21A challenges, and court reports.
- Building meaningful engagement with young people who may be difficult to engage and supporting their families and carers.
- Undertaking Carer's Assessments and supporting carers under the Care Act.
- Working closely with SEND services and understanding the role of Education, Health and Care Plans (EHCPs).
- Promoting strengths-based practice and person-centred planning.
- Working collaboratively across health, education, children's services, and adult social care to ensure seamless transitions.
- Contributing to a responsive six-day service, primarily Monday to Friday with occasional Saturday working.
We are looking for an experienced and motivated Social Worker who is passionate about improving outcomes for young people and their families.
Essential Qualifications
- Social Work Degree, DipSW or equivalent professional Social Work qualification.
- Completion of ASYE.
- Current registration with Social Work England.
- Full UK driving licence and access to a vehicle for work purposes.
- Minimum of 2 years' post-qualification experience.
- Substantial experience working with young people transitioning from Children's Services into Adult Services.
- Significant experience working within Mental Health Services.
- Knowledge and experience of SEND and Education, Health and Care Plans (EHCPs).
- Experience of Care Act assessments, care planning, safeguarding, and statutory social work practice.
You will demonstrate:
- Excellent assessment, analytical and report-writing skills.
- Strong communication and relationship-building abilities.
- Confidence working with service users, carers, families, and multi-agency partners.
- Sound knowledge of relevant legislation and statutory responsibilities.
- Strong organisational skills and the ability to manage competing priorities.
- The ability to work effectively in challenging and complex situations.
- A commitment to anti-oppressive and strengths-based practice.
- The ability to represent and advocate for the views and wishes of service users and carers.
- Experience of working independently while contributing positively to a team environment.

Grade G-N
- Up to 10 years' service: 28 days annual leave
- 10 years' service: 31 days annual leave
AMHP qualification is welcomed but not essential.
Qualified and warranted AMHPs receive:
- Additional payment of £82 per AMHP shift
- Approximately 25-30 shifts available annually
Working Pattern
Full-time hours:
- Monday: 8:30am - 5:00pm
- Tuesday: 8:30am - 5:00pm
- Wednesday: 8:30am - 5:00pm
- Thursday: 8:30am - 4:30pm
- Friday: 8:30am - 4:00pm
